PCOS Diet

Polycystic Ovary Syndrome (PCOS) is a condition that affects 5-10% of women of childbearing age. PCOS is associated with: irregular menstrual cycles, abnormal hair growth or loss, abdominal obesity, elevated insulin levels, elevated testosterone levels, polycystic ovaries, dark patches of velvety skin on neck, arms, breasts or thighs, acne, and infertility. Nearly 50% of women with PCOS are overweight or obese. Improving your diet and exercise program by making lifestyle changes may reduce your risk for developing chronic diseases associated with PCOS such as diabetes, heart disease and endometrial cancer. Fertility is higher and pregnancy is healthier when a woman has a BMI (body mass index) between 20-25. BMI is calculated by comparing the ratio of height and weight (BMI = weight in kg divided by height in meters squared). The maximum ideal BMI is 25 kg/m2. Currently there is no scientific evidence to support one particular diet for PCOS. Evidence-based recommendations suggest that women with PCOS should focus on balance and moderation. Recommended lifestyle changes include: ·         Weight loss of 5-10% if overweight or obese in 3 months. ·         Decreased caloric intake if weight loss is desired. ·         Decreased intake of enriched carbohydrates. ·         Increased fiber intake including fruits, vegetables, and beans. ·         Decreased fat intake, particularly saturated fat. ·         Smaller, more frequent meals (every 3-4 hours) to help control blood glucose levels. ·         Balanced meals including complex carbohydrates, plant based protein, and less than 10% plant based fat. ·         At least 150 minutes of moderate or vigorous activity per week for diabetes prevention. KEY POINTS TO REMEMBER Consume a wide variety of whole foods ·         Foods in their most natural form (fresh, frozen, or dried). ·         Fruits, vegetables, beans, legumes, whole grains, fish, lean meats (only once a week), nuts (walnuts), and seeds (chia) should be eaten daily. Aim to eat 25-30 grams of fiber per day ·         Introduce fiber gradually to your diet to minimize gastrointestinal upset. ·         Choose vegetables such as broccoli, lettuce, celery, cabbage, cucumber, parsley, radishes, spinach, turnips, and watercress. ·         Choose legumes such as fresh cooked kidney beans, lentils, black eyed peas, chickpeas, and lima beans. ·         Choose fruits such as strawberries, raspberries, grapefruit, apples, cherries, peaches, pears, and plums. ·         Eat them as snacks, salads, sandwich fillings, in smoothies, soups, and casseroles. Limit sugars and enriched carbohydrates ·         Complex carbohydrates take longer than enriched carbohydrates to digest and absorb. Enjoy 1-2 servings per meal. A serving is equivalent to ½ cup whole wheat pasta, brown rice, or beans/lentils; 1 slice whole wheat bread (or 1 ounce of bread); 1 serving of healthy cereal; ½ cup cooked oatmeal; ½ cup starchy vegetables like peas, corn, or potatoes; ½ medium baked potato; or 1 piece (or 1 cup) of fruit. You do not have to limit non-starchy vegetables — they’re super low in calories and high in fiber. ·         Choose whole grains instead of enriched grains (brown rice, whole wheat breads and pasta, oats, barley, etc.). Look for the word “whole” as the first ingredient on the Nutrition Facts Label. ·         If insulin resistance or diabetic, talk to a Registered Dietitian about the appropriate number of carbohydrate servings you need per day. Limit salt intake (aim for less than 2400 milligrams of salt per day) ·         Use lemon juice, mustard, vinegar, pepper, herbs and spices instead of table salt to season foods. ·         Limit foods such as cured and smoked meats, salted nuts, canned and processed vegetables, meats, marinades and sauces. ·         Eliminate intake of processed foods. Choose unsaturated fats ·         Avoid saturated fats by eliminating all dairy products and spreads, and reduce white meat and fish, and lean cuts of red meat. ·         Look for unsaturated oils such as olive, grape seed, and avocado oil and use these oils in very small amounts. ·         Use nuts rich in monounsaturated and polyunsaturated fats such as almonds, walnuts, pecans, flax seeds, sunflower and pumpkin seeds. ·         Eat fish two to three times a week. Fish such as tuna, salmon, herring, sardines, and trout can improve heart health. ·         Bake, grill, broil, boil, and steam instead of frying them. Eat protein and and/or fat with every meal or snack ·         Protein has a stabilizing effect on the sugar released from carbohydrates into the blood. ·         Suggested protein can be found in lean meats (limited), fish, poultry, beans, nuts and seeds. How stress can have a negative impact in reproduction and how acupuncture can lower stress

Scientist know that stress boosts levels of stress hormones-glucocorticoids such as cortisol which in turn inhibits the release of the main sex hormone, gonadotropin releasing hormone (GnRH) which in turn will lower sperm count, have a negative impact on ovulation and sexual activity. New research also shows that stress also increases brain levels of a reproductive hormone named gonadotropin inhibitory hormone, or GnIH. This is a small protein hormone a so called RFamide-related peptide (RFRP) inhibits the production and release of GnRH, therefore inhibiting reproduction. Therefore, stress can have a negative impact in reproduction in two ways, and at times the negative impact can have a double negative effect on reproduction ( Sanders et al; 2009) Many studies have shown that acupuncture can lower stress. In a study it was shown that patients who received acupuncture before and after embryo transfer had experienced lower stress level and higher pregnancy rate (64.7%) compare to group who did not receive acupuncture (42.5%) (Balk et al; 2010). Also in clinical settings many patients report a relaxed state after acupuncture treatments. Although the research on effects of acupuncture on cortisol level have been inconclusive, Eshkevarai et al (2012) has shown that Electro acupuncture can suppress the sympathetic peptide, neuropeptide Y (NPY), has been shown to be elevated in chronic stress and is hypothesized to contribute to the physiological stress response. Chinese herbs and Acupuncture for menopausal symptoms

Menopause is a natural process which all women face; it can be challenging right of passage and at times an uncomfortable process for many women. The result indicated that there was a significant decrease in the climacteric scale score and visual analogue scale after 6 months of herbal treatment. There was significant increase in plasma GM-CSF levels and a significant decrease in plasma FSH levels after 6 months of herbal administration (1). Another study, measured the effectiveness of Chinese herbal medicine for reducing premenopausal depression. After 12 weeks of treatment with Chinese herbs, patients reported lower scale of depression according to Hamilton Depression Scale, lower FSH level and increased estrogen level (2). Acupuncture for sleep problems

Acupuncture for Insomnia Insomnia and sleep disturbances are commonly encountered clinical problems affecting 40% in certain population. Insomnia may represent impaired sleep induction, the presence of sleep fragmentation, or non-restorative sleep. Pharmacological treatment: 1. Medications such as benzodiazepine and tricyclic antidepressants. Acupuncture and pelvic pain

At Acuwellness Center, we are specialized in women’s health and treating gynecological problems using Traditional Chinese medicine including Acupuncture and Chinese herbs. Acupuncture for dysmenorrhea

Few recent studies show how Acupuncture can be effective in decreasing the discomfort associated with painful periods (Dysmenorrhea). In a particular study researchers had 66 young women who experienced painful periods without an underlying cause such as endometriosis or a fibroid. In this study, the first group received electro acupuncture on hands and ankles at specific acupuncture points (LI4, SP6). The second group received electro acupuncture on non-acupuncture points. After 8wks of treatment the first group noticed a significant decrease in pain while the secondary "sham" acupuncture group received no benefit ( Wu LL, Su CH, et al; 2012) Studies such as above are a step in right direction to show how acupuncture and Chinese Medicine can be effective in treating common gynecological problems. Menstrual pain Introduction:Many women experience menstrual pain, especially when they are in the their teens. In most cases, menstrual pain does not indicate a more serious problem, although sometimes it can be associated with endometriosis or uterine fibroids (non-cancerous tumors in the uterus).The medical term for menstrual pain is primary dysmenorrhea.Primary dysmenorrhea usually starts 2 - 3 years after the first period, as a woman begins to ovulate regularly. Pain usually starts a day or two before menstrual flow, and may continue through the first 2 days the period. They include beans, almonds, and dark green leafy vegetables (such as spinach and kale). •Eat foods that are high in antioxidants, including fruits (such as blueberries, cherries, and tomatoes), and vegetables (such as squash and bell pepper). •Avoid refined foods, such as white breads, pastas, and sugar. •Eat fewer red meats and more lean meats, cold-water fish, tofu (soy, if no allergy), or beans for protein. •Use healthy cooking oils, such as olive oil or vegetable oil. •Some women find that adding soy milk to their diet helps relieve menstrual pain. •Eliminate trans-fatty acids, found in commercially baked goods such as cookies, crackers, cakes, French fries, onion rings, donuts, processed foods, and margarine. •Avoid caffeine, alcohol, and tobacco. •Drink 6 - 8 glasses of filtered water daily. •Exercise at least 30 minutes daily, 5 days a week.The following supplements may also help relieve menstrual pain: •Omega-3 fatty acids, such as fish oil, about 6 g per day, to help lower inflammation. A few studies have found that women who took fish oil had less menstrual pain than those who took placebo. Omega-3 fatty acids may increase the risk of bleeding, especially for people who take blood-thinning medication. Ask your doctor before taking omega-3 fatty acids. •Calcium citrate, 500 - 1,000 mg daily. Calcium is needed for healthy bones, and may also help reduce menstrual pain because it helps maintain muscle tone. Evidence isn' t clear, however. One study found that calcium did reduce menstrual pain, but in another study it seemed that calcium reduced premenstrual pain but did not help after a woman' s period started. Calcium citrate is the form of calcium that your body absorbs most easily. •Vitamin D, 400 IU daily, helps your body use calcium and may reduce inflammation. •Vitamin E, 500 IU daily, may help reduce menstrual pain. In one study, 100 young women took either 500 IU of vitamin E or placebo for 5 days (2 days before and 3 days after their periods started). Those who took vitamin E reported less pain than those who took placebo. •Magnesium, 360 mg daily for 3 days on the day before menstruation starts. One study that used this dosage found that it reduced menstrual cramps in women who took it. A few other preliminary studies have also suggested that magnesium may help reduce menstrual pain. Too much magnesium can cause diarrhea and lower blood pressure. If you have digestive problems or heart disease, ask your doctor before taking magnesium.HerbsHerbs are generally available as standardized dried extracts (pills, capsules, or tablets), teas, or tinctures/liquid extracts (alcohol extraction, unless otherwise noted). Mix liquid extracts with favorite beverage. Dose for teas is 1 - 2 heaping teaspoonfuls/cup water steeped for 10 - 15 minutes (roots need to be steeped longer).No scientific studies have been done on whether these herbs can reduce menstrual pain, but some have been used traditionally for that purpose. Some researchers think these herbs may have estrogen-like effects. Women who have a history of hormone-related cancer, who are taking hormone replacement therapy, or who have a bleeding disorder or are taking blood-thinning medication should ask their doctor before taking these herbs: •Chaste tree or chaste berry (Vitex agnus castus) standardized extract, 20 - 40 mg daily before breakfast. •Cramp bark (Viburnum opulus), taken as a tea. Boil 2 tsp. dried bark in 1 cup water then simmer for 15 minutes; drink 3 times per day. •Black cohosh (Actaea racemosa) standardized extract, 20 - 40 mg two times a day. •Evening primrose oil (Oenothera biennis) standardized extract, 500 - 1000 mg daily, as a source of gamma linolenic acid (GLA). Some studies -- but not all -- have found evening primrose oil to be effective at relieving symptoms of premenstrual syndrome (PMS). •Turmeric (Curcuma longa) standardized extract, 300 mg three times a day, for inflammation. Turmeric can increase the risk of bleeding, so ask your doctor before taking it.Physical MedicineThe following methods may help relieve pelvic pain: •Castor oil pack. Apply oil directly to skin, cover with a clean soft cloth (for example, flannel) and plastic wrap. Place a heat source (hot water bottle or heating pad) over the pack and let sit for 30 - 60 minutes. For best results use 3 consecutive days in 1 week. •Contrast sitz baths. Use two basins that you can comfortably sit in. Sit in hot water for 3 minutes, then in cold water for 1 minute. Repeat three times to complete one set. Do one to two sets per day, 3 - 4 days per week.AcupunctureThe National Institutes of Health recommend acupuncture as either a supplemental or alternative treatment for menstrual pain. In a well-designed trial involving 43 women with dysmenorrheal, women treated with acupuncture showed a dramatic reduction in both pain and the need for pain medication.Acupuncture has become a popular treatment for menstrual pain. Acupuncturists treat people with dysmenorrhea based on an individualized assessment of the excesses and deficiencies of energy (called qi) located in various meridians. In the case of dysmenorrhea, a qi deficiency is usually detected in the liver and spleen meridians. Moxibustion (a technique in which the herb mugwort is burned over specific acupuncture points) is often added to enhance needling treatment, and qualified practitioners may also recommend herbal or dietary treatments.Acupressure is also effective at reducing pain.
